Your first appointment

All new patients are sent a secure SMS to their mobile phone that contains our “New Patients Registration Form”. Please fill in and submit.

If you are unable to complete via our mobile registration method, the form is available from reception.

Please remember to bring

Please remember to bring to your first appointment:

Photo ID – Drivers Licence or Passport.
Your Medicare, DVA, Seniors or Concession Card
List of any medications, allergies or recent test results

This information allows our staff to better asses your medical needs and provide you with the most appropriate medical advice and treatment.

Claim your Medicare rebate online

You can claim your Medicare rebate via electronic claiming after your doctor’s appointment at our offices. Your rebate will go straight into your bank account.

To submit a Medicare claim online, you must be registered, have your Medicare card and your doctor’s account – Find out more here or ask one of our friendly reception team for more information.

Payment options

Please pay your account at the conclusion of your appointment by cash, cheque, EFTPOS, MasterCard or Visa.
